Procedures to obtain information

Submit a request by mail, fax, email or in person to include the following information:

• Requestor contact information:  Name, Address, Phone Number, Fax Number, Email Address
• Requestor preferred format for information: paper copy, electronic copy, inspection only
• Preferred method of delivery:  U.S. Mail, Email, Fax, Pick-Up
• Adequate description and details about the information requested in order to enable the governmental body to accurately identify and locate the information requested
